Thank
you for choosing Bankcard Liberty as your credit card
processing provider. Click here for the GOLD package
application you need to fill out to start you on your way to
accepting credit cards for your Cookie Lee business.
While you complete the first page with your
information, please note the following:
1.
DBA: This
will be (Your Name) For Cookie Lee.
This will appear on your customers’ receipts, as well
as their credit card statements at the end of the month.
2.
DBA Phone#: This
number will appear on your customers’ receipts, as well as
their credit card statements at the end of the month.
They will use this number to call you with any
questions about their purchase.
3.
Bank Reference Phone #:
I need this to contact your bank to verify your account
information.
4.
If you have filed for bankruptcy within the last seven
years, please provide the month and year, as well as a copy of
the bankruptcy discharge papers.
5.
Business Trade Suppliers:
As you can see, Cookie Lee is one of your references.
If you are a member of Costco or Sam’s Club, please
write that in. (As
a sole proprietor, you can have a personal account and use it
to buy business supplies.)
If not, please put the name, address and phone number
of the person who recruited you.
This is just to verify that you are doing business with
at least two sources.
Please
sign in the following areas:

Page 2, American Express, Signature and Date (if you
want to accept Amex)
2.
Page 2, Discover, Signature and Date (if you want to
accept Discover)
3.
Page 2, bottom left, Individual Guaranty, Signature and
Date
When all pages are signed, please fax the following to 310.878.0152:
1.
Both application pages
2.
Copy of voided check – (must be light enough to read
pre-printed name and numbers at bottom of check)
3.
Copy of your Driver’s license – (suggestion:
enlarge and make lighter for easier reading)
4.
Signed Wireless Addendum
Remember, we don’t have any monthly minimums, which means we don’t
require you to do any specific volume at any time. You
will also receive a monthly statement, outlining your
transaction and fees. In review, your regular account
fees will be as follows:
1.
1.75% + 30c per transaction
2.
$5 Account Maintenance fee (Monthly)
3.
$20 Wireless Airtime fee (Monthly; unlimited, no extra
long-distance or roaming charges)
4.
$27.50 Semi-annual fee (debited 2-3 months after
signup)
*Note:
If you should close your account for any reason within the
first year, you will be debited for a $150 early termination
charge. Also, please let me know if you are
interested in the online service. This allows you to
view your transactions online 24/7. We provide this
service for an additional $7.50 per month.
Your checking account will be debited for $119 ($69 - account
activation’ $50 – wireless activation) as soon as we
receive your signed copies. It usually takes 3-5
